Guidelines
- Enter the child's name exactly as per the Corporation Birth Certificate
- For Catholics / Christians, ensure the child's name on the Baptism Certificate matches the Corporation Birth Certificate
- Incomplete / incorrect sheets will be rejected after verification
- Bring the child's original birth certificate for verification on the day of application submission
Required Documents
- One recent passport-size photo of your child to be attached to the form
- One recent postcard-size group photo (Father, Mother, and child)
- Corporation Birth Certificate (with name of the child entered) — photocopy
- Baptism Certificate (Catholics/Christians) — photocopy
- Aadhaar Card — photocopy
- Address Proof — photocopy
How to Submit
During the weekdays (Monday to Friday) from 2.00 p.m. to 3.30 p.m. at Sacred Heart Nursery School's registration desk with an application fee of Rs.500/- only (by cash). Please bring the child's original birth certificate for verification on the day of application submission.
